Bitrates are your encoding settings; CDN prices are entered by you. Player buffering and retransmissions are covered only by the overhead percentage. Everything is calculated in your browser; nothing is sent or probed on the network.
Average bitrate is weighted by viewer share; peak bandwidth = peak viewers × average bitrate; monthly data = average viewers × bitrate × hours.
1,000 viewers on a 1080p/720p/480p/360p ladder need about 4.2 Gbps at peak.
